Pediatric to Adult Health Transition for Adolescents with Medical Complexity: The Role of Family and Social Determinants of Health

‍Youth with serious illness are increasingly moving into adolescence and young adulthood. The process of pediatric-to-adult transition and the experiences of youth and their families during this transition has not been fully explored. In this webinar, we will review evidence to date on the pediatric-to-adult health transition for adolescent and young adults with serious illnesses and describe potential impacts of social determinants of health.
